Follow-up outcome assessments at 30 days, six months and one year. All fees included in the price of the hearing aid. Hearing aid consultations free of charge.

Direct primary care membership

Direct Primary Care membership for $100 per month including: Direct access to provider 24/7, same-day/next-day scheduling, unlimited visits with 45-minute appointments, at-cost in-house labs and prescriptions, personalized longevity-focused approach, and house calls when needed

Enrollment Fee: $100 (Will apply only once to the first invoice received in addition to the monthly membership fee.)
