Individual DPC membership starts at $109 per month; Family plan $179 per month (includes 2 domestic partners and their children). Includes unlimited in-clinic and telehealth visits, annual physical, direct physician access, same or next-day appointments, discounted medications, vaccines, labs, and imaging. Home visits and telehealth visits for members are discounted/free as specified (in-clinic visits free for DPC members; home visits $150 per member).
