We have created a form that will help us address your patient’s sleep disordered breathing even more quickly. By including the Sleep History form below with any referral, this will help us skip an initial assessment appointment and enter the pre-authorization process for scheduling an in-lab or at-home sleep study. This dramatically increases the speed at which we can address an individual’s sleep disordered breathing. It also saves the individual from making an additional office visit with a copay.
